With Your Listing, You Are Extending Your Reach And Strengthening Our Community
Add your business information below to be listed in the UANP Directory and become a member of the UANP. Thousands of patients will be able to customize their search to find the precise services you offer. The annual membership fee is $125 and can be paid on the next page after entering your info to the directory below. Your listing will not be live until your membership is paid. You will be able to add or update your information at any time.
You can also mail a check payable to UANP to 242 S 400 E, SLC, UT 84111, however, this is not a preferred method as it will delay processing and requires additional steps to add your business listing.
Connect your Google My Business account to quickly auto-populate some of the fields below, including your business description, hours, address, images, videos, phone number, website, and email. This is a one-time import and will not sync any future updates from your account. Your Google Reviews will not sync to your account either. Any imported information can be modified or deleted as desired. You can also manually input information to each field below without connecting your Google My Business, however, you will not have access to this auto-populate feature after you submit your listing.
Make sure to complete all required fields that are denoted by the “*” symbol. We also recommend that you have all information prepared and saved in a separate Word document as a backup before submitting your listing. You will be able to Update Your Listing after it is submitted whenever you like. If for some reason your screen freezes, refresh your screen without pushing the back arrow.
